General Release

I understand that roadside assistance and vehicle handling may involve a risk of incidental damage, particularly where prior damage, corrosion, altered components, concealed defects, wheel lock issues, electronic malfunctions, or other vehicle conditions exist. By signing below, I authorize the requested service and acknowledge the condition of the vehicle as presented at the time of service. I hereby release and hold harmless Ozark Roadside, LLC, the servicing technician, any contractors or subcontractors providing service, and the dispatching platform, including but not limited to Agero and its parent companies, subsidiaries, affiliates, agents, and representatives, from any claims or liability for incidental damage not caused by negligence or willful misconduct. Any damage resulting from pre-existing conditions, concealed defects, or circumstances beyond the reasonable control of the service provider shall remain the responsibility of the vehicle owner or customer.

Lockout Section

I acknowledge that lockout service may involve use of entry tools and that weather stripping, trim, glass, switches, interior handles, or locking systems may already be worn, damaged, or fragile. I am authorizing the use of these tools on my vehicle and understand the risks associated to using these tools such as scratches on the B pillar, torn weatherstripping, sprung doors, interior door scratches and anything else that could be associated to the use of such tools to perform the job.

Tire Service Section

I acknowledge that lug nuts, wheel studs, wheels, wheel covers, TPMS components, and related hardware may be rusted, swollen, seized, overtightened, cross-threaded, damaged, or otherwise compromised prior to service. Due to these conditions, removal of a wheel may require additional force or specialized tools and may result in further damage to already compromised components including lug nuts, studs, wheel covers, caps, or related hardware.

Chrome-capped lug nuts: Caps may be cracked, spinning, loose, or rusted underneath prior to service. These caps are fragile and may need to be removed during normal tire service. I accept full responsibility for any pre-existing or incidental damage to chrome caps.

I understand that roadside tire service is intended only to temporarily return the vehicle to a drivable condition so the vehicle may be moved to the nearest qualified tire repair facility. Temporary spare tires are typically designed for speeds not exceeding 50 mph and distances generally not exceeding approximately 40 miles from the location of installation, depending on the manufacturer specifications and the condition and age of the spare tire.

I acknowledge that spare tires deteriorate with age and that most temporary spares have a service life of approximately 4–6 years. I accept that Ozark Roadside, LLC, any contractors, dispatching platforms, including Agero and its parent companies, subsidiaries, affiliates, agents, representatives, and the technician cannot be responsible for the condition, age, prior use, or failure of a spare tire or any customer-supplied equipment used during the service.

If lug nuts or studs are broken, rounded, seized, cross-threaded, or otherwise unable to be removed using standard methods, I authorize the technician to use extractor tools or other necessary removal methods to remove the wheel and install the spare tire so the vehicle can be safely returned to the roadway. I understand that these procedures may cause additional damage to the affected lug nuts, studs, or related components and I authorize the technician to perform these procedures as necessary to complete the service.

I acknowledge that I have been informed of these risks and I authorize the servicing technician to perform the tire service requested.

By approving extractor or removal methods, I authorize the servicing technician to attempt wheel removal using standard extraction procedures. I understand that this may further damage already compromised lug nuts, studs, wheel covers, caps, or related components and I accept responsibility for those conditions where they are pre-existing or incidental to the approved removal process.

Fuel Delivery Section

I understand that adding fuel to a vehicle carries risks if the wrong fuel type is requested or if there are pre-existing mechanical issues. I confirm that I have requested the correct fuel type for my vehicle and accept responsibility for any issues arising from incorrect fuel information provided by me.

Jump Start Section

I understand that battery, charging, wiring, or electrical issues may exist before service and that a jump start does not correct underlying mechanical or electrical faults. I acknowledge that a portable jump box equipped with surge protection, reverse polarity protection, and other safety features will be used to attempt to start the vehicle. I understand that the servicing technician or contractor, Ozark Roadside, and its affiliates are not responsible for damage resulting from pre-existing electrical conditions or vehicle system failures beyond their control.

Out of Scope Service Acknowledgment

I understand and acknowledge that the service being performed is not the original service dispatched through Agero and is outside the scope of the service request sent to Ozark Roadside, LLC.

I understand that this out of scope service is being provided solely by the servicing technician as a separate arrangement between myself and the servicing technician. I acknowledge that Ozark Roadside, LLC, Agero, and Agero’s parent companies, subsidiaries, affiliates, agents, representatives, contractors, subcontractors, and dispatching platforms are not responsible for, and have no involvement in, this out of scope service.

I understand that any payment, dispute, damage claim, complaint, warranty issue, or other issue related to this out of scope service must be addressed directly with the servicing technician who performed the service, and not with Ozark Roadside, LLC, Agero, or any of their affiliates or related entities.

By approving this service, I acknowledge that I am voluntarily requesting and accepting a service outside the original dispatched call and that I do so at my own discretion.

Final Vehicle Inspection Acknowledgment

I have had the opportunity to inspect my vehicle and have found it to be in good working order and that no damage as a result of the service provided has occurred to the vehicle including doors, door frame, paint, glass, window tint, rims, lug nuts / studs, body or underbody of the vehicle. I also acknowledge that all equipment such as jacks, lug wrenches, lug nut keys and special tools belonging to me or the vehicle have been returned in good working order.

If this box is checked, the driver must explain in Additional Notes why the customer declined to sign the waiver.
